As your Louisville family dentist of choice, we want to make your first visit to our office as convenient as possible and we have included our new patient forms in an electronic format so that you can fill them out at your convenience prior to your visit. To do this, click on the Patient Registration Button at the top of the screen. Type in your first name, last name, appointment time, and click on submit. New patients complete all three forms and established patients please complete the LDA Medical and Dental History only.
